In compliance with Ordinance No. 20, s. 2026 (VISITA Ordinance of the City of Baguio) and Republic Act No. 10173 (Data Privacy Act of 2012), I hereby voluntarily provide my information for registration in the VISITA System.

I understand and agree that:

1. Collection of Information

   The VISITA System shall collect my submitted data, including but not limited to the registered name of the accommodation establishment, business address, contact details, and supporting permits, for the purpose of accreditation, monitoring, and regulation of tourism-related establishments.

2. Purpose and Legal Basis

   The processing of my data is necessary for the implementation of the VISITA Ordinance, including coordination with the Business Permits and Licensing Office (BPLO) and other relevant government agencies.

3. Public Disclosure of Business Information

   I consent that the following business-related information may be published in the VISITA platform and other official government channels for transparency and tourism promotion:

   • Name of the establishment
   • Type/classification
   • General business address or location
   • Licensing and accreditation status
4. Confidentiality of Personal Data

   Personal data relating to individuals, including private contact details and identification information, shall be treated as confidential and shall not be disclosed without my consent, unless authorized or required by law.

5. Data Sharing

   My submitted data may be shared with authorized government offices, including the City Government of Baguio, BPLO, and the Department of Tourism, strictly for validation, enforcement, and regulatory purposes under the VISITA Ordinance.

6. Data Retention

   My data shall be retained only for as long as necessary to fulfill the purposes of the VISITA System and in accordance with applicable laws, rules, and regulations.

7. Declaration and Consent

   I certify that:

   • I am authorized to represent the establishment
   • All information provided is true and correct
   • I fully understand and consent to the collection, use, and processing of my data as stated herein

By proceeding with registration, I signify my informed and voluntary consent under the VISITA Ordinance and applicable data privacy laws.
